Legend Biotech adds five-year durability to CARVYKTI’s rapidly growing commercial story

Legend Biotech reports five-year CARVYKTI remissions as sales rise 50%, strengthening the case for earlier use in multiple myeloma.

Legend Biotech has added an unusually long durability argument to the commercial case for CARVYKTI, with new five-year data showing that half of a small group of earlier-line multiple myeloma patients remained alive, progression-free and free from additional anti-myeloma treatment after a single infusion. The results arrive as CARVYKTI is already moving deeper into earlier treatment settings and generating rapidly expanding global sales, making long-term remission increasingly relevant to both prescribing decisions and the economics of the high-cost cell therapy. Median progression-free survival reached 60.5 months and five-year overall survival was 69.2%, while median overall survival had not been reached. The 20-patient Phase 2 cohort is too small to establish that CARVYKTI can routinely produce five-year treatment-free remission, but the durability reinforces the strategic rationale for moving the therapy earlier in the multiple myeloma treatment sequence.

The clinical update also lands at an important point in Legend Biotech’s financial evolution. CARVYKTI generated approximately $657 million in worldwide net trade sales during the second quarter, up 50% from a year earlier, while increasing earlier-line adoption helped the company report its first quarter of company-wide profitability. Legend Biotech ended June with roughly $965 million in cash and time deposits, giving it substantial resources to expand CARVYKTI while investing in next-generation cell therapies.

Five-year treatment-free survival could strengthen the economic argument for using CARVYKTI earlier

CARTITUDE-2 Cohort A enrolled patients with relapsed or refractory multiple myeloma who had received only one to three previous treatment lines and whose disease was refractory to lenalidomide. After a median follow-up of approximately 60.7 months, 10 of the 20 patients remained alive and progression-free without maintenance therapy or another anti-myeloma treatment.

That outcome matters commercially because CARVYKTI’s value proposition differs fundamentally from drugs that must be administered continuously. Autologous CAR T-cell therapy requires an intensive upfront treatment process, but patients who experience prolonged remission may spend years without another anti-myeloma regimen.

For payers and treatment centers, long-term treatment-free intervals could therefore become an increasingly important part of the cost-benefit discussion. Multiple myeloma is generally managed through sequential therapies as patients relapse, potentially exposing them to combinations of immunomodulatory agents, proteasome inhibitors, monoclonal antibodies, bispecific antibodies and other treatments over many years.

A single intervention capable of delaying that sequence for several years could reduce subsequent drug exposure and healthcare utilization for patients who achieve deep durable responses. The current CARTITUDE-2 results do not provide a formal health-economic analysis, but five-year treatment-free disease control in half of the cohort gives Legend Biotech and Johnson & Johnson stronger long-term evidence with which to support CARVYKTI’s positioning.

The findings also complement randomized evidence from CARTITUDE-4, which helped expand CARVYKTI into patients whose disease had progressed after at least one prior treatment line. Earlier access has materially increased the addressable population and is already influencing real-world utilization, with Legend Biotech reporting that second- through fourth-line use accounts for more than 70% of United States CARVYKTI volume.

Earlier-line adoption is becoming a major growth engine for CARVYKTI sales

CARVYKTI’s commercial trajectory shows why the five-year findings matter beyond clinical conferences. Worldwide net trade sales reached approximately $657 million during the second quarter, representing 50% year-over-year growth and a 10% sequential increase. United States sales climbed 32% to $472 million, while sales outside the United States surged 128% to approximately $185 million.

Legend Biotech attributed United States sequential growth partly to accelerating earlier-line adoption. The therapy is available through more than 150 authorized treatment centers in the United States, with about 40% classified as community hospitals, while the global network has expanded to 348 treatment sites across 19 markets.

That expansion is important because CAR T-cell therapies historically faced capacity and access constraints that limited their commercial reach. Each CARVYKTI treatment requires patient-specific cell collection, manufacturing, quality testing and reinfusion, making supply-chain execution fundamentally different from conventional pharmaceutical manufacturing.

Extending treatment into community hospitals could broaden the referral network and reduce dependence on large academic centers. International expansion provides another growth lever as manufacturing capacity and reimbursement become established in additional markets.

The five-year results may support that expansion by helping physicians assess whether the complexity of CAR T treatment is justified earlier in the disease course. A patient with multiple available conventional therapies may view an intensive one-time procedure differently if there is credible evidence that some recipients can remain treatment-free for five years or longer.

The limitation is that CARTITUDE-2 Cohort A cannot tell clinicians how frequently that outcome will occur across broader practice. Only 20 patients were treated, and the study did not randomly compare CARVYKTI with another therapy. Patient selection could have influenced the unusually durable outcomes, making larger controlled evidence more informative for determining how broadly the result applies.

Long-term safety remains essential as CARVYKTI moves into patients with more treatment options

Earlier use also raises the safety threshold. Patients who have received only one or two previous treatment regimens may have several effective alternatives available, meaning physicians must weigh CARVYKTI’s potential for prolonged remission against the risks associated with lymphodepletion and CAR T-cell therapy.

CARVYKTI’s prescribing information includes serious risks involving cytokine release syndrome, neurologic toxicity, prolonged and recurrent cytopenias, infections, secondary malignancies and other immune-mediated complications. Long-term neurological monitoring has received particular attention following reports of movement and neurocognitive toxicities, including rare parkinsonism-like syndromes.

The new CARTITUDE-2 follow-up did not identify new CAR T-related neurological toxicities during the extended observation period. The study did report an additional hematologic malignancy and additional deaths involving disease progression and a new malignancy, reinforcing the need for continued long-term surveillance even years after infusion.

Those risks do not negate the five-year efficacy signal, but they remain part of CARVYKTI’s commercial ceiling. Greater penetration into earlier treatment lines will depend partly on whether treatment centers become more comfortable managing the therapy and whether longer-term evidence continues showing a favorable balance between durable remission and severe toxicity.

CARVYKTI growth is pushing Legend Biotech toward a different financial profile

Legend Biotech is increasingly moving away from the financial profile of a conventional development-stage biotechnology company. Second-quarter total revenue reached $387.5 million, including $326.1 million in collaboration revenue associated primarily with CARVYKTI. Operating income reached $57.7 million compared with a $21.9 million operating loss a year earlier, while net income was $33.2 million compared with a $125.4 million loss in the prior-year period.

Adjusted net income reached approximately $63 million, and management expects adjusted profitability to continue through the second half of 2026. The shift reflects operating leverage from CARVYKTI as sales expand more quickly than portions of the company’s cost base.

The distinction between CARVYKTI trade sales and Legend Biotech revenue is important. Johnson & Johnson and Legend Biotech jointly commercialize the therapy, so the $657 million worldwide sales figure is not booked entirely as Legend Biotech revenue. Legend Biotech recognized its economic share through collaboration accounting, including approximately $326 million of collaboration revenue during the quarter.

Legend Biotech also strengthened its balance sheet through an equity offering that generated approximately $212 million in net proceeds, ending June with about $965 million in cash, cash equivalents and time deposits. Management has indicated that the company expects those resources to extend beyond the point at which it reaches sustained company-wide profitability.

That financial position gives Legend Biotech greater freedom to invest beyond CARVYKTI. The company is developing an in vivo CAR T platform intended to generate engineered immune cells directly inside patients, potentially avoiding the individualized ex vivo manufacturing process required by CARVYKTI. Those programs remain early, but CARVYKTI’s commercial growth can increasingly finance development rather than leaving the company wholly dependent on external capital.

Legend Biotech stock remains well below recent highs despite CARVYKTI’s commercial momentum

Legend Biotech shares entered the September 25 session at $19.06 after gaining 3.31% in the previous trading session. The stock remained well below its 52-week high of $37.50 and below the $29.35 price at which the company completed its June equity offering.

That performance creates an interesting contrast with the operating business. CARVYKTI sales are rising rapidly, international adoption is accelerating and Legend Biotech has crossed into quarterly profitability, yet the share price has remained under pressure.

Investor caution appears to reflect several factors beyond CARVYKTI efficacy, including manufacturing scalability, competition in multiple myeloma, dilution from the June financing and the company’s recent leadership transition. Ingrid Zhang took over as chief executive in September as Legend Biotech entered a period in which commercial execution is becoming as important as clinical development.

The five-year CARTITUDE-2 results strengthen one part of that investment case by providing unusually long evidence that CARVYKTI can deliver durable disease control when used earlier. They do not resolve manufacturing or valuation questions, and the tiny study population means the data should not be treated as proof that half of all comparable patients will remain progression-free for five years.

What the results do show is that prolonged treatment-free remission after one CAR T infusion is possible in some earlier-line patients. If larger datasets continue supporting that pattern while CARVYKTI sales and manufacturing capacity expand, Legend Biotech’s commercial story could increasingly shift from whether CAR T can become a major multiple myeloma treatment to how large the earlier-line opportunity can ultimately become.
