TACTI-004 investigation gives Immutep a possible explanation as efti prepares for 2027 restart

Immutep finds efti manufacturing differences after TACTI-004 and refocuses development on head and neck cancer and soft tissue sarcoma.

Immutep is attempting to rebuild the investment case for eftilagimod alfa after identifying manufacturing differences that could help explain the unexpected failure of its pivotal lung cancer trial and narrowing future development to two potentially more manageable oncology opportunities. The biotechnology company now plans to prioritize PD-L1-negative head and neck squamous cell carcinoma and neoadjuvant soft tissue sarcoma while returning efti production to the 200-liter manufacturing scale used across several earlier successful clinical studies. New registration-directed trials are targeted to begin in the second half of 2027, subject to manufacturing, regulatory, partnering and funding decisions. The strategy gives Immutep a potential route back toward late-stage development after the March collapse of TACTI-004 devastated its valuation, although the company has not yet established that manufacturing differences caused the Phase III failure.

That distinction remains crucial for investors. Immutep has identified subtle structural differences, including variation in N-glycan characteristics, between efti produced at the 2,000-liter scale used exclusively in TACTI-004 and the 200-liter material used in previous Phase I and Phase II studies. The findings create a plausible hypothesis for further investigation, particularly because TACTI-004 also produced a markedly different immune-activation profile, but the root-cause analysis is still underway.

Returning efti to 200-liter manufacturing could reduce one major uncertainty after the Phase III setback

TACTI-004 had been expected to establish efti as a major first-line lung cancer asset. Instead, an independent data monitoring committee recommended discontinuing the Phase III study for futility in March 2026 after the experimental combination underperformed the control arm, forcing Immutep to reassess both the program and its broader development strategy.

Subsequent analysis made the result even more striking. Among 173 patients included in the futility analysis, the objective response rate reached 42.9% for patients receiving efti with standard treatment compared with 55.1% for the placebo combination. The efti group failed to demonstrate superiority across PD-L1 subgroups, contrasting sharply with earlier studies in which the drug had generated encouraging response and survival signals.

Immutep now says its investigation has not identified clinical-trial design, randomization, baseline patient imbalances, general trial conduct or safety findings as explanations for the result. Instead, the company has found structural differences between the product used in TACTI-004 and earlier clinical material, including a subtle N-glycan difference that may be biologically relevant.

Ten good manufacturing practice batches produced at the 200-liter scale were previously used across studies including TACTI-mel, TACTI-002 and INSIGHT-003. TACTI-004, by comparison, relied exclusively on efti manufactured at the larger 2,000-liter scale, and Immutep has now contracted another 200-liter production run as it prepares for possible clinical development beginning in the second half of 2027.

The manufacturing decision is strategically important because efti is a biologic, meaning relatively subtle changes in production can potentially affect molecular characteristics and biological activity. Glycosylation patterns can influence the behavior of protein therapeutics, so a reproducible relationship between manufacturing scale and immune activation would be clinically meaningful.

However, identifying a difference is not equivalent to proving causation. Immutep still needs to show whether those structural variations materially changed efti’s potency or biological behavior and establish a manufacturing process capable of consistently producing the intended product. Regulators would also be expected to examine comparability closely before any future registration program advances.

Head and neck cancer and soft tissue sarcoma now offer Immutep narrower routes back toward registration

The development reset represents a major change from Immutep’s previous emphasis on first-line non-small cell lung cancer. Future registration-directed work will initially focus on head and neck squamous cell carcinoma in patients with PD-L1 Combined Positive Score below one and the neoadjuvant treatment of resectable soft tissue sarcoma.

The head and neck cancer opportunity is supported by Phase IIb TACTI-003 data. In patients with PD-L1 CPS below one, efti combined with Merck & Company’s Keytruda produced median overall survival of 17.6 months. Immutep has emphasized the unmet need in this subgroup because PD-L1-negative patients have more limited chemotherapy-free immunotherapy options.

The United States Food and Drug Administration has already granted efti Fast Track designation in first-line head and neck cancer, while previous interactions with the agency have been characterized by Immutep as constructive. That regulatory history helps explain why the company sees this narrower population as a more attractive near-term route than immediately attempting another broad lung cancer Phase III program.

Soft tissue sarcoma provides a second potential route. In the investigator-led EFTISARC-NEO Phase II study, efti with radiotherapy and Keytruda produced median tumor hyalinization or fibrosis of 51.5% among 38 evaluable patients, significantly exceeding the trial’s prespecified 35% target and historical levels of roughly 15% associated with radiotherapy alone. Disease-free and overall survival data remain immature, meaning longer follow-up will be important before the clinical significance of the pathological response can be fully established.

The FDA granted efti Orphan Drug Designation in soft tissue sarcoma in April 2026. Taken together, the regulatory designation, positive Phase II result and relatively concentrated patient population give Immutep another possible pathway for advancing efti without immediately returning to the scale and expense of the abandoned lung cancer program.

The strategy also limits near-term capital exposure. Developing two more focused indications allows Immutep to rebuild evidence around efti while the manufacturing investigation continues, potentially reducing the risk of committing large sums to another expansive Phase III trial before product consistency is better understood.

Immutep cash runway and Dr. Reddy’s partnership provide time for the efti development reset

Immutep ended June with approximately A$68.87 million in cash, cash equivalents and term deposits, including A$63.67 million in cash and A$5.20 million in short-term deposits. The company has implemented cost reductions following the TACTI-004 discontinuation and currently expects its financial runway to extend well into the first half of 2028.

That timing matters because Immutep does not expect the next registration-directed efti trials to begin until the second half of 2027. The existing runway could therefore carry the company through manufacturing work, regulatory discussions and initial preparations, although larger late-stage programs would likely require additional funding or partnership support.

Dr. Reddy’s Laboratories remains an important component of that strategy. Under the companies’ agreement, Dr. Reddy’s Laboratories holds exclusive efti development and commercialization rights across licensed territories outside major markets retained by Immutep, while Immutep remains eligible for up to $349.5 million in potential milestones plus royalties and retains global manufacturing rights. Immutep repaid $10 million of the original upfront payment after TACTI-004 was discontinued, but Dr. Reddy’s Laboratories remains supportive of the revised development approach.

Immutep has also disclosed preliminary discussions with other parties regarding the proposed development pathway. Additional partnering could become important if the company decides to move both priority indications rapidly into registration-focused studies while continuing to develop IMP761, its separate LAG-3 agonist program for autoimmune disease.

The broader pipeline offers some diversification from efti. IMP761 produced favorable safety and pharmacodynamic findings in its first-in-human Phase I program, including evidence of reduced inflammatory responses and T-cell activity, with pharmacokinetics supporting dosing approximately once every four weeks. Efti nevertheless remains the asset most directly connected to Immutep’s near-term oncology valuation.

Immutep stock response shows cautious optimism but TACTI-004 damage still dominates sentiment

Investors in Australia responded positively to the strategy update, with Immutep shares rising after the company disclosed the manufacturing findings and revised clinical priorities. The reaction suggests the market sees value in the possibility that TACTI-004 may have reflected a product-specific manufacturing issue rather than definitive failure of efti’s underlying mechanism.

That optimism remains modest compared with the destruction of shareholder value following the original Phase III failure. Immutep’s Nasdaq-listed American depositary receipts closed at approximately $0.325 on September 10 after falling more than 8% during the session, and the shares remain dramatically below their pre-TACTI-004 levels. The stock lost more than 80% on March 13 when the trial discontinuation was announced, illustrating how heavily the company’s valuation had depended on the lung cancer program.

The latest update therefore represents the beginning of a potential rehabilitation of the efti investment case rather than a full recovery. A manufacturing explanation would be strategically favorable because it could leave the drug’s underlying immune-stimulation hypothesis intact, but investors will need stronger evidence that the 200-liter material consistently reproduces the biological activity observed in earlier studies.

The next meaningful milestones should include completion of the root-cause analysis, characterization of the new 200-liter manufacturing batch, regulatory clarity on the head and neck cancer and soft tissue sarcoma programs, and confirmation that the planned trials can begin during the second half of 2027. Progress across those areas could gradually reduce uncertainty, while an inability to connect manufacturing characteristics with biological performance would leave the TACTI-004 result more difficult to explain.

For Immutep, the strategic reset creates a credible route forward without erasing what happened in Phase III. The company has narrowed its clinical ambitions, preserved enough cash to investigate the manufacturing question and selected two indications supported by encouraging earlier evidence. Whether that approach restores meaningful value will depend on demonstrating that efti can once again produce consistent immune activation and clinical activity when manufactured under the process Immutep now plans to take forward.
